Alprostadil (Caverject) Self-Injections, and how to administer them

General information: Alprostadil is an FDA-approved vasodilating prostaglandin (a hormone-like compound). It is widely prescribed when oral medications like Viagra and Cialis fail or are not effective. It works by relaxing the blood vessels in the penis which results in improved blood flow leading to an erection. It also works by suppressing noradrenaline which results in dilation of the cavernosal arteries resulting in engorgement of the pens which leads to an erection.

Alprostadil has advantages over oral medication because it has a faster onset, the ability to adjust the dose, fewer interactions, and fewer contraindications. Injected erectile medications bypass other bodily systems and work directly on the penis therefore it minimizes potential side-effects like headaches or gastrointestinal disturbances. Alprostadil is commercially available as Caverject and at the right dose it has 40 to 70% efficacy rate. Alprostadil is safe and effective in the treatment of erectile dysfunction, however, there are risks and side effects involved with its use and must be discussed with your healthcare provider.

Candidates for Alprostadil: Patients who have failed on oral ED tablets, have low testosterone, diabetes, prostate issues, neurological/spinal injury, and prostate cancer. General injectable erectile medications are appropriate for most patients who have been precluded from taking oral ED drugs.

Storage Information: Medication can be kept in the refrigerator but preferably must be stored in the freezer. Keep out of the reach of children. Throw away any unused medication after the beyond-use date.

How to administer Alprostadil injections

Alprostadil is administered as an intracavernous injection into the side of the penis by wiping the intended injection site with a sterile alcohol wipe. Use a half-inch 27-30g needle to inject into the side of the penis avoiding visible veins.  Alternate the sides of the penis for each use. Alprostadil dose is individualized and must be dosed as a titration.  If there is no response after the first injection, wait 24 hours and increase the dose by 2.5-10mcg. Once the appropriate injection dose is established, the patient may use the drug not more than 3 times a week with a minimum of 24 hours between doses.

Bimix Injections including Alprostadil

General information: The name suggests it contains 2 components which are injected into the penis (Papaverine and Phentolamine).  These two drugs are generally combined and highly effective in treating erectile dysfunction when oral medications like Cialis or Viagra have failed. Two drugs with different mechanisms of action allow them to work complimentary to one another producing a synergistic effect. Bi-mix is safe and effective in the treatment of erectile dysfunction, however, there are risks and side-effects involved with its use, and must be discussed with your healthcare provider.

Papaverine is a smooth muscle vasodilator that works via complex mechanisms of action which include inhibition of PDE enzyme which results in increased cGMP in the penile tissue. This ultimately relaxes blood vessel walls which will improve blood flow in the penis resulting in an erection.

Phentolamine is a long-acting adrenergic alpha receptor blocker that works by encouraging smooth muscle relaxation and results in increased blood flow when injected into the penis.

Generally, injectable erectile medications are appropriate for most patients who have been precluded from taking oral ED drugs.
